Emergency Cards Please note that your child will receive 2 Blue Emergency Cards to be filled out. Please fill them out completely with at least 2 emergency contacts and phone numbers. Students will be released from school ONLY to person named on the Emergency Card. If student’s home address has changed over the summer please fill out the official change of address / phone# form in the main office, and include proof of new address (utility bill). Home address or phone number will not be changed based on Emergency Card information. It is Parent’s / Guardian’s responsibility to submit accurate and up to date information to school. |
